Terms & Conditions
The client acknowledges that the information provided to him or her by Doug Cook, of Doug Cook RD is designed to meet his or her personal dietary and nutritional needs. It is not suitable for any other individuals and will not be transferred, copied, or sold to another person. The client takes full responsibility for his or her health, choices and progress on his or her nutrition care plan.
In order to benefit from the treatment plan suggested by Doug Cook of Doug Cook RD, the client acknowledges that it is his or her responsibility to inform either his or her physician or Doug Cook of Doug Cook RD of any changes he or she makes in the application of the treatment plan. It is the client’s responsibility to report any side effects or problems immediately and to make the necessary adjustments to the treatment plan with the help of his or her physician or Doug Cook of Doug Cook RD. Doug Cook of Doug Cook RD is not responsible for any complications that result from my failure to comply and adhere with the above.
The client agrees to have the Registered Dietitian keep records of his or her visits and to file these in a secure and appropriate place per the Privacy Act of Ontario and the requirements of the College of Dietitians of Ontario and the Regulated Health Professionals Act of Ontario. Information may be shared at the client’s request with other health professionals/practitioners that he or she deems appropriate.
SCHEDULLING & CANCELLATION
A 24 hour notice is required to reschedule or cancel all appointments; any rescheduled or cancelled appointment with less than a 24 hour notice will be billed in full; if the appointment is a follow-up session as part of a package, the follow-up session will be lost. All fees are non-refundable and non-transferable.
Payments are due in full prior to initial session via paypal, credit card, cheque made payable to Doug Cook, or by e-money transfer.
